Supporting Our Local Teams
We provide sports physicals tailored to the specific requirements of local schools, including:
- Argyle ISD: Argyle High School, Middle School, and Gibson Middle School. We are familiar with the Rank One Sport digital submission process used by Eagle athletics.
- Liberty Christian School: Providing the required documentation for Warrior athletes to upload to FinalForms.
- Surrounding Areas: Serving student-athletes from Northlake, Robson Ranch, and the Denton area.
What the UIL Exam Includes
Per Texas UIL and TAPPS requirements, our evaluations cover:
- Medical History Review: Comprehensive check of past illnesses, surgeries, and family cardiac history.
- Physical Examination: Checking vital signs (blood pressure/pulse), vision, heart, lungs, and musculoskeletal health.
Appointment Checklist
- Forms: Please bring the current 2026-27 UIL Physical Evaluation form.
- Signatures: Ensure the Medical History and Parent/Guardian sections are signed before your visit.
- Digital Records: After your visit, we can provide the completed paperwork for you to upload to your school's athletic portal (Rank One or FinalForms).
Note: A sports physical is a specific-purpose exam for athletic clearance and does not replace a full annual wellness check-up.
